Short-Term Disability Insurance

How It Can Help
When you buy Voluntary Short-Term Disability Insurance, your protection includes:

  • Monthly income benefits of 60% of your pay, up to $5,000 per month, for six months or two years, depending on your employment classification (see below)
  • No premiums are payable while you are totally disabled

Who Can Be Covered
You may choose coverage for yourself as an eligible Botsford employee. (Full-time salaried employees are not eligible to enroll in Voluntary Short-Term Disability coverage.)

Special Features
Coverage features include the following:

  • Benefits are paid directly to you, and you can use the money any way you wish
  • Benefit start and end dates coordinate with your Botsford disability coverage:

    For Part-Time and Full-Time Hourly, Non-Benefit Eligible Employees
    Benefits begin after 14 days of total disability
    Maximum benefit period of six months

    For Full-Time Hourly Benefit Eligible
    Employees

    Benefits begin after 180 days of total disability
    Maximum benefit period of two years

Enrolling for Coverage
You can enroll for coverage after you have completed 3 months of service as a full-time benefit eligible employee or as a part-time management employee, or 12 months of service as an eligible part-time non-management benefit eligible employee or as a full-time non-benefit eligible employee.

Employees can apply for coverage from August 1st to August 18th 2006. New hires and employees that do not elect coverage during this enrollment period will have an opportunity to enroll the following August 2007 during the voluntary benefit annual enrollment period. With respect to the effective date, employees that enroll from August 1st to August 18th 2006 will have an October 1, 2006 policy effective date.

Short-Term Disability benefits are underwritten by Trustmark Insurance Company, Lake Forest, Illinois. Refer to Plan Series DI-902 and other optional riders for your state for exact terms and provisions.
