Who
Can Be Covered
You may choose coverage for yourself only, you and your spouse,
you and your children, or you, your spouse and your children.
Special Features
Coverage features include the following:

A specific expense is not required for the lump sum benefit payout;
you can use the money any way you wish

You can buy optional features that would automatically increase
your benefit amount every year for five years, and you can choose
the option that would pay for an annual screening test (such as
a mammogram or PSA test) each year, up to $100.
Enrolling
for Coverage
You can enroll for coverage after you have completed 3 months of
service as an eligible full-time employee, or 12 months of service
as an eligible part-time employee.
Employees
can apply for coverage from August 1st to August 18th 2006. New
hires and employees that do not elect coverage during this enrollment
period will have an opportunity to enroll the following August 2007
during the voluntary benefit annual enrollment period. With respect
to the effective date, employees that enroll from August 1st to
August 18th 2006 will have an October 1, 2006 policy effective date.

Illness Insurance Including Cancer is underwritten by Trustmark
Insurance Company, Lake Forest, Ilinois. For complete details of
